A 6-MAM urine test is a qualitative screening that detects the presence of 6-monoacetylmorphine (6-MAM) in urine. 6-MAM is a unique metabolite of heroin (diacetylmorphine) and its presence confirms heroin exposure specifically, rather than general opiate use.
This test detects the presence of 6-monoacetylmorphine (6-MAM), the primary metabolite that is exclusive to heroin. While heroin is rapidly metabolised into morphine, 6-MAM serves as a definitive marker that distinguishes heroin use from other opiate exposure.
The test provides a qualitative result indicating whether 6-MAM is present above the assay cut-off level. Due to the very short detection window of 6-MAM, timely sample collection is critical.
6-MAM testing provides definitive confirmation of heroin exposure. Standard opiate tests detect morphine, which can originate from heroin, codeine, or poppy seed consumption, making it impossible to confirm the specific source.
The presence of 6-MAM eliminates ambiguity and is considered the gold standard for confirming heroin use in clinical, forensic, and workplace settings.
Testing is relevant when confirmation of heroin exposure is needed, particularly when a standard opiate screen is positive for morphine. Due to the very short detection window of 6-MAM (approximately 2 to 8 hours in urine), testing should be performed as soon as possible after suspected exposure.
