6 months without alcohol: what changes and what your blood shows

After 6 months without alcohol, the fastest blood markers are long back to normal in most people. CDT within a few weeks, gamma-GT in weeks to months, and the MCV in about four months. Your hair tells a longer story: the last three centimetres show roughly the last three months. Six months is therefore the point where blood and hair can both fit abstinence.

The biggest changes come in the first month. In a British study, systolic blood pressure fell by 6.6 percent after one month without drinking (Mehta, 2018). Insulin resistance fell by 25.9 percent, weight by 1.5 percent. Below we set out, milestone by milestone, what changes and what you can measure.

We're writing this for people who don't just want to feel better. They want to see it. Often because an employer, a court or the CBR is asking for proof.

6 weeks without alcohol: what changes first?

After 6 weeks without alcohol the fastest markers have already turned. CDT has a half-life of about 15 days (Stibler, 1991). After six weeks a raised value is back under the 1.7 percent upper limit in many people. Gamma-GT falls over the same period, although the pace depends on how high it was.

Blood pressure and sleep respond quickly too. In the study by Mehta and colleagues, blood pressure and insulin sensitivity had measurably improved after four weeks. Halving an intake above two drinks a day lowered systolic pressure by 5.5 mmHg on average (Roerecke, 2017). A drink here is a standard drink of about 10 grams of alcohol, the Voedingscentrum's measure.

2 months without alcohol: blood pressure, insulin and weight

After 2 months without alcohol the gains of the first month are usually held. Mehta's study found 6.3 percent less diastolic pressure after one month. Weight fell 1.5 percent. Those who keep going hold those numbers or build on them.

The fitness-to-drive guideline gives gamma-GT a window of several weeks. After two months a raised gamma-GT has therefore often normalised.

One caveat we think matters: gamma-GT also rises with medication, excess weight and liver disease. A gamma-GT that's still high after two months doesn't have to be alcohol. Discuss such a result with your GP.

3 months without alcohol: MCV and the first 3 centimetres of hair

After 3 months without alcohol the MCV comes into view, the average size of your red blood cells. A red blood cell lives about four months. An alcohol-enlarged MCV therefore only falls once the old cells have been replaced.

A Japanese study followed men with a strongly raised MCV, 110 fl or more (Yokoyama, 2017). In the risk group that share fell from 50.5 to 28.7 percent in four weeks. After eight weeks it fell further.

We often call MCV the forgotten marker.

After 100 days without alcohol there's something else measurable: hair. Scalp hair grows about a centimetre a month. The last three centimetres at the scalp therefore show roughly the last three months. An EtG hair test on that segment can make a drink-free period visible.

What does 6 months without alcohol do to your body?

After 6 months without alcohol, CDT, gamma-GT and MCV are back to normal in most people. Blood pressure and insulin sensitivity have been improved for months by then.

The brain partly recovers too. A German MRI study measured the brain's ventricles 6 to 9 months after stopping (Wobrock, 2009). In people who stayed abstinent or drank less, they were 5 to 6 percent smaller.

Six months is also the point where blood and hair can tell the same story. The blood markers look back weeks, the hair months. Anyone who has both measured has two independent sources.

What does a year without alcohol do to your body?

After a year without alcohol, little usually changes in the blood values anymore. They were normal after six months. What's added is the meaning of that year. For the CBR, a recurrence-free period of one year is the yardstick. Someone found unfit for alcohol misuse can be found fit again after it.

For your body, a year is the point where the slow effects join in. Weight, blood pressure, sleep and the brain ventricles from the MRI study. The Gezondheidsraad has good reason to advise no alcohol, or at most one drink a day.

How long without alcohol for a clean liver?

"Clean" isn't a lab word. But the liver values that react to alcohol usually normalise in weeks to a few months. Gamma-GT returns within several weeks, the guideline says.

Whether the liver itself recovers depends on how much damage there was. A raised gamma-GT that doesn't fall deserves a conversation with your GP.

How long does your body need to recover from alcohol?

Count on weeks for the blood markers and months for the MCV and the hair. The brain changes in the MRI study take six to nine months. Those are study averages, not a promise for one person.

Anyone who drank heavily for a long time may need longer. Anyone who keeps having complaints should talk to their GP.

How do you show 6 months without alcohol?

With a combination of blood and hair. A CDT test shows the last weeks, an EtG determination in hair the last months. Together they cover the period you want to show, each with its own window.

Two things are worth knowing. Under the 2026 guideline, the CBR assessment itself uses CDT and gamma-GT, not hair analysis. A private test doesn't replace that assessment, and the CBR doesn't adopt our result.

And a negative result doesn't prove abstinence in an absolute sense. It fits abstinence, and that's what you show with it.

Say two people are both on day 180 without alcohol. One has only blood drawn, the other blood and hair. Both CDT values sit below 1.7 percent. Only the second can also show the months before. A hair segment below the EtG threshold of 7 pg/mg.

The psychiatrist brings their own lab results. What you had measured yourself helps you walk into the interview without surprises. In our experience that's the real gain.

What are the effects of 4 months without alcohol?

After four months CDT and gamma-GT are normal in most people and the MCV starts to fall. The red blood cells from before you stopped have largely been replaced by then. The blood-pressure and insulin gains of the first month remain. The hair by now shows well over three months without alcohol.

What if a value is still raised after 6 months?

Then alcohol is usually not the explanation. Gamma-GT and MCV have other causes, from medication to a vitamin B12 deficiency. Discuss the result with your GP. The BIG-registered doctor who reviews your Zuivertest result also adds context.
