GP urgent care, your own GP or a lab test: where should you go?

The Netherlands has three routes for three kinds of questions. For acute complaints there is the GP urgent-care post (huisartsenspoedpost), for non-urgent medical questions your own GP, and for an objective measurement of alcohol or drugs there is the laboratory test. If you know the criteria, you pick the right door within a minute. This article lines them up.

Do you need care right now?

In a life-threatening situation, always call 112 immediately. Think of severe shortness of breath, persistent chest pain, sudden loss of function or unconsciousness.

For complaints that cannot wait for a regular appointment but are not life-threatening, the huisartsenspoedpost is available outside office hours. The route is the same everywhere: call the regular number of your own GP. The answering machine immediately gives you the telephone number of the nearest urgent-care post. This works throughout the Netherlands, including public holidays.

Every region has its own post. In the eastern Achterhoek, for example, it is the Spoedpost Huisartsen Oost-Achterhoek at the Streekziekenhuis Koningin Beatrix in Winterswijk, and in the Ede, Wageningen, Veenendaal and Barneveld region the Huisartsenspoedpost Gelderse Vallei. Both work by telephone appointment only.

When is your own GP the right route?

For all non-urgent complaints you start with your own GP: fatigue, abdominal complaints, poor sleep, concerns about your drinking, or a question about an earlier result. Your GP knows your file, can refer you onwards and can order blood tests that are reimbursed under the basic insurance when medically indicated.

That explicitly includes drinking behaviour.

If you are worried about your own use, a conversation helps more than a number. The Dutch GP guideline uses a questionnaire and a conversation for this, and deliberately not a blood value. Which signals count is described in recognising problem drinking.

When does a laboratory test without a referral fit?

There are situations where the central issue is not a complaint but a demonstrable value. Think of a fitness-to-drive procedure at the CBR, an employer or agency asking for certainty, or a period in which you want to document that you are not drinking or using. In those situations a self-reported story is not enough; only a measurement counts.

For those evidential questions there is laboratory testing without a referral. You pay for the test yourself and every result is reviewed by a BIG-registered physician.

The boundary is sharp. If you have physical complaints, your question belongs with your GP, not with a self-paid test. A lab value without medical context answers an evidential question, not a care question.
