3-MMC in urine: detection, isomers and what the product really contains

3-MMC is, according to a Dutch study from 2026, a synthetic cathinone and an isomer of mephedrone (4-MMC): 3-MMC has the same atoms as mephedrone, with one group in a different place. According to that study, 3-MMC became popular in the Netherlands because of its stimulant effect and because it was legal at first. According to the Trimbos Institute, 3-MMC can bring cheerfulness, an upbeat feeling, less anxiety and more self-confidence.

According to the Trimbos Institute, 3-MMC can also bring muscle and jaw tension, a dry mouth, restlessness and sleeplessness, and craving, the urge to use again and again. Only a test shows whether 3-MMC has been in the body; in a laboratory, that is possible in blood and urine.

Unilabs publishes no cut-off or unit for 3-MMC in urine and gives "n.aant.", not detectable, as the expected result of its MMC panel. This 3-MMC test looks for 3-MMC in urine.

3-MMC is a synthetic cathinone, a stimulant related to cathinone from the leaves of the khat shrub. The 3-MMC test looks for 3-MMC in urine; Unilabs gives "n.aant.", not detectable, as the expected result.

3-MMC: what this test measures

A 3-MMC test in urine looks for a substance that is chemically almost the same as 2-MMC and 4-MMC: the three differ only in the place of one group. Unilabs looks for the three together in one MMC panel, run by the Apotheek Haagse Ziekenhuizen (AHZ) with LC-MS/MS, a form of mass spectrometry.

According to a 2025 study, such positional isomers must first be separated for a mass spectrometer to recognise them reliably.

3-MMC: why this value matters

At DIMS, a national network of 33 drug-checking services, only 10% of samples sold as 3-MMC still contained 3-MMC in 2024, against 85% in 2021, according to a Dutch study from 2026. According to that study, substitutes such as 2-MMC and 3-CMC and more potent substances such as DMP (di(methyl)pentylone) and NEP (N-ethylnorpentedrone) took its place.

In that 2026 Dutch study, DMP and NEP were associated with more severe adverse reactions, such as anxiety, paranoia, heart and blood vessel problems and insomnia.

3-MMC: when is testing worthwhile?

How long 3-MMC stays detectable in a laboratory urine test after use has never been studied according to the Trimbos Institute; it is probably a number of days. Urine collected within 1 hour of drug use can still show nothing, according to the NVKC.

3-MMC: symptoms of a high or low value

Low Levels

A negative 3-MMC result means no 3-MMC was found in the urine. The result can also be negative after using a product sold as 3-MMC, because at DIMS, a national network of 33 drug-checking services, only 10% of samples sold as 3-MMC still contained 3-MMC in 2024, against 85% in 2021.

High Levels

According to the NVKC, the Dutch society for clinical chemistry, a positive 3-MMC result in urine must be confirmed with another analysis method, because a positive drug test can have far-reaching consequences.

According to a 2025 study, of 15 Belgian oral-fluid samples that were positive for mephedrone (4-MMC) with a non-selective method, only 1 contained mephedrone with the method developed in that study, while 6 samples contained 3-MMC and 12 contained 2-MMC.

3-MMC: causes of a low or high value

What lowers the value

  • The product was not 3-MMC

    In the Dutch drug-checking programme DIMS, only 10% of samples sold as 3-MMC still contained 3-MMC in 2024, against 85% in 2021.

  • Urine collected too soon after use

    In a urine drug test, it can happen according to the NVKC that nothing can be detected yet when the urine was collected within 1 hour of use.

What raises the value

  • 2-MMC or 4-MMC

    According to a 2025 study, 2-MMC and 4-MMC are chemically so close to 3-MMC that a mass spectrometer cannot easily tell them apart unless they are separated first.

3-MMC: lifestyle and this value

Combining 3-MMC with medicines, alcohol or other drugs increases the chance of unpleasant side effects and of needing medical help, according to the Trimbos Institute. Because 3-MMC is a new drug, many risks are not yet known, according to the Trimbos Institute.

3-MMC: frequently asked questions

Does an ordinary amphetamine test find 3-MMC?

An ordinary amphetamine test does not always find substances related to 3-MMC: in a 2021 study, an amphetamine immunoassay, a test that works with antibodies, reacted to mephedrone (4-MMC) at less than 1%, and the authors write that such tests can lead to false-negative results. That study gives no figure for 3-MMC itself.

Can a self-test detect 3-MMC?

Yes, according to the Trimbos Institute there are urine self-tests that react to 3-MMC. Those self-tests test for the group of methcathinones, which also includes 3-CMC, 2-MMC and 4-MMC, and according to the manufacturer give a positive result 1 to 3 days after use.

Is 3-MMC addictive?

Yes, according to the risk assessment of the Dutch coordination point for assessing and monitoring new drugs (CAM), as summarised by the Trimbos Institute, 3-MMC is very addictive because of strong craving. According to the same summary of the risk assessment of 3-MMC, more and more people need help from a doctor or addiction doctor.
