Health declaration for your driving licence: when and how

A health declaration is a digital CBR form on which you answer questions about your health so the CBR can assess whether you're medically fit to drive. You complete it yourself through Mijn CBR using DigiD. It was formerly called the "Eigen verklaring". The cost is a small fixed fee.

What is a health declaration?

A health declaration is a CBR questionnaire about your health in relation to driving fitness. You complete the form yourself, sign it digitally, and the CBR decides from your answers whether a medical assessment is needed. This form used to be called the "Eigen verklaring".

The questions cover things like your eyes, heart, use of alcohol or medication, and any condition that might affect your driving. You answer them with yes or no, sometimes with a short note.

Completing it doesn't automatically mean you'll be examined. Often the declaration itself is enough, and sometimes the CBR asks for extra checks.

So the declaration is a first step, not a final verdict.

When do you need a health declaration?

You need a health declaration when you turn 75 and your licence expires, when you have a condition or medical event that might affect driving, or when the CBR asks for one after an alcohol or drug matter. The table below lists common situations and whether a declaration applies.

Not everyone renewing a licence needs one. Under 75, it usually only applies when there's a health reason or the CBR requests it.

A declaration can also be needed when getting a first licence, for example if you have to report a condition or medication use.

How do you apply for a health declaration?

You buy and complete a health declaration digitally through Mijn CBR. You log in with DigiD, answer the health questions, pay a small fixed fee, and sign the form digitally. Most municipalities still offer a paper version too.

Have your DigiD ready before you start. Without DigiD you can't submit the digital declaration.

Answer the questions honestly and fully. Incorrect answers can later affect how your driving fitness is assessed.

What happens after you submit it?

After you submit, the CBR reviews your answers. Sometimes the declaration is enough and you're told you're fit. In other cases the CBR refers you to a medical assessment or a specialist, for example an eye doctor or another relevant examination. The turnaround time varies by situation.

For an assessment, a doctor completes a report that goes to the CBR. The CBR then decides about your driving fitness based on it.

If you're referred after an alcohol or drug matter, a blood-value check can be part of the procedure. A raised CDT value, for example, might point to prolonged heavy alcohol use.
