ng/ml stands for nanograms per milliliter. It's a concentration unit that shows how much of a substance sits in a sample. On a drug test result, ng/ml reports the measured amount. The lab compares that number to a cut-off value. Here's what it may, and may not, mean.

What does ng/ml mean?
ng/ml means nanograms per milliliter, a unit of concentration. A nanogram is one billionth of a gram. A milliliter is one thousandth of a liter. Together they show how much of a substance sits in a small amount of fluid. So it's a ratio, not a total dose.
You can picture ng/ml as a measure of density in fluid. Imagine you dissolve a pinch of dye in a glass of water. A second glass with more dye gets a higher value per milliliter.
Labs use ng/ml because substances in urine or blood appear in very small amounts. Other units would give awkwardly large or tiny numbers. With ng/ml the value stays readable.
On your result you'll usually see the substance, the measured value, and the unit. That unit is nearly always ng/ml for these tests. So you know which scale the lab reports in.
A higher ng/ml value means a higher measured concentration. A lower value means a lower concentration. On its own, the number says nothing about right or wrong.
A unit is a ruler, not a verdict.
How does ng/ml relate to the cut-off value?
The cut-off value is a fixed limit set in advance, also in ng/ml. The lab compares your measured value to that limit. If the concentration sits above the cut-off, the lab calls the result positive. If it sits below, the result is negative. The limit helps separate noise from a real signal.

Different substances have different cut-off limits. A low limit may pick up more signals. A higher limit aims for clearer results. The lab follows recognized agreements here.

Does a high value show how much someone used?
Not directly. A high ng/ml value tells you about the concentration in the sample, not about the amount someone used. Many factors shape that concentration. Think of fluid intake, metabolism, time since possible contact, and how diluted the urine is. So a number doesn't equal a dose or impairment.

- time since possible contact with a substance
- the properties of the specific substance
Some substances build up in the body and stay measurable longer. Others clear faster. So a similar situation can give different values.
The timing of the sample may also play a role. Early in the day the urine can be more concentrated. Later, after drinking a lot, the same substance can look diluted. So values can differ from moment to moment.
Impairment or fitness to drive doesn't follow from an ng/ml number alone. That needs other tests. A doctor can weigh the context a number misses.
A lab value describes a sample, not behavior.
How do you read your own value?
